"Sale Price Change FEE Form"

By Signing & Dating this page the Customer agrees to the following.

1)   When "1st Choice Mall" receives this "Sale Price Change FEE Form", signed and dated by the Customer, this "Sale Price Change FEE Form" shall be a legally binding Contract between "1st Choice Mall" and the Customer.
2)   Abide by all of the "Policies" below.    (a policy with a   **   requires a FORM.)
  1) After receiving this FORM, if it is completed accurately and when the customers check has
      cleared, the items listed on a separate paper and signed by the customer will have the Sale Price
      changed within 5 days at a cost to the customer of  $5.00 (each item) "PAID-IN-FULL".
  2) ** All items can be removed  FREE  at any time before the end of the one (1) year listing.
  3) ** Any Removed item can be relisted by paying an "ITEM   RELISTING   FEE".
  4) ** New item or picture can replace any listed by paying an "ITEM   REPLACEMENT   FEE".
  5) ** An item's Text can be changed ONLY one (1) time  FREE  if the item's Text is incorrect.
  6) ** An item's Text can be changed a second time by paying a "TEXT   CHANGE   FEE".
  7) ** Any item's Sale Price can be reduced  FREE  ONLY one (1) time in the one (1) year listing.
The Customer MUST check the listing on the Internet for accuracy & inform "1st Choice Mall" of  errors.

3)   Picture(s) & Text Page(s) received by "1st Choice Mall", from the customer, will be kept by "1st Choice Mall" as proof of the listing information received by "1st Choice Mall".

On a separate paper print every Sale Price to be changed (Name, Category, and Price).  Sign and date the paper. Mail the paper, check & this FORM to the address listed to the left.
